Overview

Customize your form by adding questions for use in your business and creating a more personalized experience for your customers.

Add a question to a form

After you create a blank form (Settings > Forms > Add Form), the next step is to customize the questions for that form. To customize form questions, click the + Add new item button on the Add a Form screen. The following window appears, where you can select a question type:

  • Header: Allows you to enter a question to display on the form.

  • Description: Allows you to enter an additional description/explanation of the question.

  • Required: Check this field to make the question required for your technicians or customers to answer before they complete/submit the form.

  • Type: Use this dropdown to select the type of question you want to create. The field below Type describes the question type. See the table below for a detailed explanation of question types.

Text

Allows you to enter short-sentence responses.

Note: There is a 1000-character limit.

Number

Allows you to enter numeric response (whole or decimal numbers).

Checkboxes

Allows you to select multiple options from a given list

After the Checkboxes type is selected, the Option 1 box appears below the Type field. The text in the box can be replaced with your answer.

  • To enter additional options, press Enter on your keyboard or select the Click to add option button to enter as many answers as you need.

  • Answer choices may be reordered using the up/down arrow to the left of each option.

  • Answer choices may also be deleted by clicking remove.

Radio Buttons

Allows you to select a single option from a given list.

When the Radio Buttons type is selected, the Option 1 box appears below the Type field. The text in the box can be replaced with your answer.

To remove, reorder, or alter to a horizontal view, follow the same bulleted steps as above.

To have your answer choices appear horizontally, check Place Options Horizontally.

Dropdown

Allows you to select a single answer from a pre-determined list.

When the Dropdown type is selected, the Option 1 box appears below the Type field. The text in the box can be replaced with your answer.

To remove, reorder, or alter to a horizontal view, follow the same bulleted steps as above.

Stoplight Buttons

Allows you to indicate the condition of an item on the form. Condition options include Poor (red), Fair (yellow), and Good (green).

Picture

Allows you to attach an image or video to any form (for inspections, completed work, and so on.). To upload an attachment to a form, click the Paperclip icon.

Supported media types: 

  • Image: jpg, jpeg, gif, png, webp, 3gp, rup, dng, heic, heif, tif, tiff, jfif. 

  • Video: mov, mp4

  • For more information on supported media types, see Add notes, media, and file attachments in ServiceTitan.

    Note: Capitalized file extensions won’t work in ServiceTitan. For example: JPEG files won’t work as opposed to jpeg files.

Date

Allows you to select a date from the calendar.

Signature

Allows a signature to be collected upon completion of the form. Options for Possible refusal reasons can also be provided by clicking + Add reason.

Smart Field

Smart field type selection for bill-to address in a form interface.

Allows you to select additional question types from a dropdown list of items. Smart Fields are sorted in alphabetical order.

Note: If you use equipment-related smart fields, equipment must be linked to the form. For more, see Managing forms for equipment.
