Problem
Forms do not pop up during appointments because they lack proper business unit assignment in the form settings.
Solution
Edit form business units
Go to the top toolbar and click Settings
.In the side panel, click Operations > Forms.
Find your form in the list.
Click on the form name to edit it.
Scroll down to the Business Unit section.
Click the dropdown menu in the Business Unit section.

Select the business units where you want the form to appear.
Click Save to apply your changes.
